If you have private health insurance through your employer, with strong out-of-network benefits, please check to see if your insurance plan is accepted first by filling out this form. If your health insurance is approved, your health insurance may cover part or all of our service prices. If you have a deductible, you will be required to meet this before you can use your massage therapy benefits. If your deductible has not been met, appointments that you pay out-of-pocket at Mind Your Body Wellness may count towards your deductible, but you will be required to pay an additional 10% medical billing processing fee, in addition to our regular service price. You may be required to pay coinsurance, copayment or both, depending on your insurance verification results.

The insurance verification process may take up to several business days to complete. We will contact you with the results (only by email). If approved, you will need to obtain the required letter or other documents from your doctor. According to your insurance plan and your doctor’s recommendation, we can develop a medical massage treatment plan specifically for you.

Our Cancellation Policy are still in effect for customers coming to see us with insurance. A credit card is required to hold your appointment and may be charged for breaking our cancellation policy. If your health insurance requires coinsurance, copayment, or both, you will be required to pay this. If your insurance is denied at a later date, you will be responsible for payment. Our team of insurance experts will do their best to prevent this situation during the verification process.

Only the 60-Minute Therapeutic Massage session is covered under insurance. Our specialized services including Couples Massage Session, Hot Stone LomiLomi Massage, and Reiki with Charka Balancing — are not covered by insurance.

For quick appointment booking, it is much easier to pay directly with a credit card or HSA/FSA card, but if you would like to use your insurance and if you have time for the verification process, we will be happy to accommodate you. Please be patient during the verification process.

To use your health insurance for your Mind Your Body appointment, please follow these steps:

1. Verify your insurance.
2. If approved, obtain a letter of medical necessity from your doctor.
3. Once we receive your doctor’s letter, we will contact you to schedule.

Use your HSA and FSA to Pay for Massage Therapy

We are happy to announce that Mind Your Body, Licensed Massage Therapy is now classified as a Therapeutic Massage facility under Merchant Category Code 8099. As such we are able to accept payments for services via a Health Savings Account (#HSA) or a Flexible Spending Account (#FSA).
We cannot, however, accept payments with any other health insurance plan at this time. Check with your insurance company first to verify that licensed massage therapy is covered under your specific plan. Once verified, book your appointment online using your HSA or FSA card.

In some instances, you may need a doctor’s prescription first to allow payment. We will always provide a digital receipt of your appointment, which will list your massage therapist’s license number. Please let us know this before your appointment by making a note of this when you book online under “anything we should know before your appointment”.

Cancellation Policy

At Mind Your Body, our #1 goal is to provide quality care to all our clients in a timely manner. No-shows, late arrivals, and cancellations inconvenience not only our providers, but all clients as well.

We require at least 24 hours notice for Cancellations.

Appointments are in high demand, and your advanced notice will allow another client access to that appointment time.

Appointment Cancellation

When you book your appointment, you are holding a space on our calendar that is no longer available to our other clients. In order to be respectful of everyone’s time, please call Mind Your Body as soon as you know you will not be able to make your appointment. You can easily reschedule your appointment online when you receive your email reminder 36 hours before your appointment. Also, when you receive your text reminder 24 hour before your appointment you can call the office directly.

If 24-hour notice is not given the full amount of your appointment will be charged
